Details of licence holder

Are you applying as:

This must be the company name and not the name of a director or any other person that works for the company.

This must be the individual/s named on the licence.

I/We, as named above, being the premises licence holder, are applying to vary a premises licence under section 41A of the Licensing Act 2003 for the premises described in this application.

Step 2

Premises details

Enter the premises licence. This can be found on your licence.

Upload the premises licence or relevant part of it.

Select which option you are attaching, or if you are not attaching the licence please state why.

Premises address

Can you provide a postal address, OS map reference or description of the premises?
Address

For example, a pub. Include its general situation and layout and any other information which would be relevant to the licensing objectives. It should include any activities or use which may be of concern in respect of children, regardless of whether you intend children to have access to the premises. For example, nudity, semi-nudity, films for restricted age groups, the presence of gaming machines etc.

Premises contact details

If you cannot find the address, please enter it manually

Step 3

Variation

Do you want the proposed variation to take effect as soon as possible?

(dd/mm/yyyy)

Does the proposed variation involve changing the layout of the premises?

Plans must be to scale and show all areas of the premises, including access and egress points, staircases and fixed furniture. If the plan is not suitable, we will contact you. An application is likely to be refused if the proposed variation could increase capacity for drinking on the premises, block emergency exits or routes, or impede the effective operation of a noise reduction measure.

Describe the proposed variations in detail and explain why you consider that they could not have an adverse effect on the promotion of any of the licensing objectives. Also include:

  • whether new or increased levels of licensable activities will be taking place indoors or outdoors (outdoors may include a tent) and give examples, such as whether music will be amplified or unamplified.
  • standard days and start/finish times when the activity will take place - all timings should be given in 24-hour format, e.g. 16:00 and should only include days of the week when you intend the premises to be used for the activity
  • any seasonal variations to times, for example, additional days during the summer or non-standard times for activities on particular days, such as Christmas Eve.
  • any changes to hours when the premises is open to the public.

You can provide any additional evidence to support your claim that the proposed variation is ‘minor’ and could not have an adverse impact on the promotion of the licensing objectives.

Operating schedule

What licensable activities are subject to change if this application to vary is successful?

Select all that apply.
